Local Solutions to National Improvement Priorities
Operating under the Health Quality Innovation Network (HQIN) umbrella, CCME leads health care quality improvement efforts in South Carolina, focusing on localized data-driven solutions to national improvement priorities.
At no cost to local providers, CCME helps the nursing homes, hospitals, physician practices, community health clinics, pharmacies and more that join HQIN in South Carolina to improve patient outcomes at the grassroots level while contributing to national quality goals.
These goals include reducing opioid misuse and deaths, decreasing adverse drug events, improving care coordination, reducing hospital admissions and readmissions, managing chronic disease, and improving nursing home quality.
It provides targeted assistance to nursing homes and communities in small and rural practices, those serving the most vulnerable populations and those in need of customized quality improvement.
CCME has served South Carolina for more than three decades, working alongside new and local partners on health care quality improvement.
As an HQIN member in South Carolina, we will work as your local partner to provide data analyses, quality coaching and learning opportunities that make it easier to deliver better results for your patients and implement sustainable changes in the care process. The Centers for Medicare & Medicaid Services (CMS) funds HQIN as part of its strategy for attaining national health care quality goals. Under the HQIN umbrella, CCME leads in South Carolina as the local Quality Innovation Network-Quality Improvement Organization (QIN-QIO) partner.
CCME Awarded CMS NQIIC Designation
The Centers for Medicare & Medicaid Services selected CCME as a Prime awardee of the Network of Quality Improvement and Innovation Contractor (NQIIC). This designation will allow NQIICs to bid on Task Orders over the next five years. As a Prime NQIIC awardee partnering with other Quality
Improvement contractors under the new CMS Indefinite Delivery/Indefinite Quantity (IDIQ) contract mechanism to support quality improvement efforts, each NQIIC may serve as:
- Quality Improvement Experts
- Facilitators/change agents for health care transformation by achieving bold aims at a high value
- Innovators of quality improvement
