The Carolinas Center for Medical Excellence (CCME) is committed to maintaining the trust and confidence of visitors to our Hospital Patient Safety Services ( HPSS) website. We have created this web policy to demonstrate our firm commitment to your privacy and the protection of your information.

Personal Information
You do not have to give personal information to visit the public portion of the HPSS website. This public section gives visitors the chance to read and obtain more information on HPSS projects and CCME. Additional activities on our site require you to be a registered participant of HPSS projects.

In order to access the Members Only section of the HPSS website, you will be required to register for and purchase our services. Once payment is received, a unique username and password will be assigned to your hospital. This unique username and password will be used to log in to the Members Only site and will track your movement through our site.

As part of the registration process, you will be asked to provide information about your organization and project team members.

Our primary purpose in collecting personal and organization information is to provide you with a safe, smooth, efficient, and customized experience. This allows us to provide services and features that will best meet your needs, and to customize our service to make your experience safer and easier. We only collect personal information about you that we consider necessary for achieving this purpose.

Our Use of Your Information
We use your personal information to facilitate the services you request. We use your personal information in the file we maintain about your organization, and other information we obtain from your current and past activities on the site to troubleshoot problems; measure consumer interest in our services; inform you about online and offline services and updates; customize your experience; detect and protect us against error, fraud and other criminal activity; and as otherwise described to you at the time of collection. We may compare and review your personal information for errors, and omissions and for accuracy.

When you register, you agree that we may use personal information about your organization to analyze site usage, improve our content, and customize the site 's content, layout, and services, and to improve our marketing efforts. These uses improve the HPSS site/projects and tailor it to better meet your needs, so as to provide you with a smooth, efficient, safe and customized experience while using the HPSS website.

Collection of Information
Personally identifiable information or personal information is defined as information that identifies your organization and project team members in your organization. It includes the name of your organization; names of project team members in your organization; and the organization’s and project team member’s addresses, telephone numbers, fax numbers and e-mail addresses.

We only collect personally identifiable information about your organization after you have specifically and knowingly provided such information to us. By voluntarily providing this information to us during the registration process, you will be consenting to the collection and use of your personal information by CCME.

Disclosure of Your Personal Information
If you provide personal information, such as your mailing or e-mail addresses, telephone or fax numbers, or other demographic information, CCME will not share (disclose or sell) it to third parties. This information is strictly for use for the HPSS project and will be used only by CCME staff.

Collection of Project Data
Any project data information collected from your organization through the online data collection tool, or by any other means, will only be reported in aggregate form. By voluntarily providing this information to us, you will be consenting to the collection, analysis and use of this information in aggregate form by CCME. Project data specific to your organization will only be discussed with your team.

Non-Personal Information
We analyze traffic to this website in order to understand our members’ and visitors ' needs and to continually improve our site for them. The HPSS website automatically collects and records non-personally identifiable information, which is information that does not identify you personally, but becomes available to us as a result of your visit to our site. This information includes: date and time of visit, pages visited on our site, type of computer and browser the visitor is using, and the location of visitor’s machine (by country, not street address). Many websites gather this information, though most do not notify visitors. We use this information to improve our services to you and do not share it with third parties.

HPSS Cookies
A cookie is a small piece of data that is sent to your Internet browser from a web server and stored on your computer’s hard drive. A cookie cannot read data off of your computer hard disk or read cookie files created by other websites. Cookies do not damage your system. You can choose whether to accept cookies by changing the settings of your Internet browser. You can reset your browser to refuse all cookies, or allow your browser to show you when a cookie is being sent. CCME will not be able to assist you with changing the setting on your computer, so that it may accept our cookie.

Where they are used, HPSS 's session cookies and/or session variables remember your selection criteria. You do not need to have session cookies enabled to view static web content.

Note: Regardless of the particular uses for cookies on the HPSS website, we will not share any cookie information with any third parties.
